Establish a Death Certificate

The death must be declared to the registrar of the place of occurrence, burial, residence or birth within 90 days by the head of the family, or a relative of the deceased, or by any other person who has had some knowledge of death.
This declaration must be certified by two witnesses.
In the event of death in a hospital or penitentiary establishment, the head of the establishment has the obligation to make the declaration within 90 days of the death.
With the 2011 law, sanctions are provided for any person who has the obligation to declare a death and who, without legitimate reason omits to do so, or declares a false or inaccurate act.
The penalty is a prison sentence of 1 to 3 months and a fine of 2,000 to 25,000 FCFA. This penalty also applies to the registrar who has received a declaration of death and who fails to transcribe it.

Here are the documents to provide:

- Declaration or certificate of kind of death issued by doctor or nurse
- Photocopy of NIC of the deceased or declaration of a relative or any person who knew of the death accompanied by two witnesses + photocopy of their NIC.
- 02 municipal stamps of 200 Frs Cfa for acts
After the 90-day period, it is at the request of the Prosecutor within 06 months or an additional judgment within the period of 06 months that the town hall can issue the certificate (birth, death).
